Business Visa requirements:
1. Valid passport
2. Visa Application forms to be filled in and attached 2 passport Size photos. To download the form click here
3. HIV test, Original medical report from any hospital/ medical centre.
4. Invitation letter from Azerbaijan duly attested by chamber of commerce / concerned authority
5. Bank Slip

Tourist Visa requirements:
1. Valid passport
2. Visa Application forms to be filled in and attached 2 passport Size photos. To download the form click here
3. HIV test, Original medical report from any hospital/ medical centre.
4. Hotel Voucher from Azerbaijan Tour operator
5. Bank Slip
6. Covering Letter
Visa Fees:
1. Single Entry visa- for 1 month 46 USD (Rs 2392/-)
2. Double Entry for 3 months 80 USD (Rs 4160/-)
3. Attestation per page. 50 USD.
The page must Contain the official stamp of the Ministry of External Affairs.
The Embassy will attest the page which contains that stamp and out attestation is for .....the stamp and not for the text.
4. Service Charge

**10.3 % Service tax applicable on service charge
